Reference
This page is a reading reference, not an exercise. It shows, for all ten practice cases, how the case content maps to the five social determinant domains used by Healthy People 2030, which of the selectable risk answers this project has tagged to each domain, and roughly what share of the risk marks each of those answers represents. Nothing on this page is scored, nothing here changes an attempt, and the domain a tag names is not itself part of the marking.
Every case is scored out of 100 on the same five-category rubric (version 2026-09-12.v2). Marking looks only at which individual answers you select and whether each of those answers is one the case establishes. The domain grouping on this page is a reading aid laid over those same answers: no marks are added or removed for a domain, and no domain is selected or scored anywhere in a case.
The 25 risk marks are shared evenly among the answers a case establishes, so each figure shown below is an approximate share of that one category, rounded for reading. It is not a promise of how much a finished score moves: category totals are rounded, and some answers also carry weight in the safety-and-access category, so shares can overlap and will not add up neatly. An answer the case does not establish earns nothing and subtracts 3, because recording an assumption as a finding causes real harm.
These two labels describe the domain note only, not the answers listed underneath it. Stated means the fictional case says the facts behind that domain. Possible mechanism means the domain is a reasonable thing to ask about while the case leaves it open.
A domain marked as a possible mechanism can still contain answers the case does establish, and those answers earn their marks normally — the heat case without symptoms is an example, where her own unanswered question about her blood-pressure tablet is stated in the brief even though her access to a clinician is not. Whether an answer earns marks is shown on the answer itself, by its own badge below, and never by the domain label above it.
Domain names follow Healthy People 2030 — Social Determinants of Health. The mapping of each fictional case to a domain is this project’s own reading, and has not been reviewed by anyone else.

She states food for tonight and part of tomorrow with no plan after that.
She receives WIC but not SNAP and has not been screened. The risk is an unchecked eligibility question, not a guaranteed benefit.

She states this. The right response is helping her get a clear answer from financial counseling, not predicting eligibility.

She states the repair cost is more than she can afford this month.
She states it. It is why the cooling has stayed broken for two days.

Two days of food left, and he is uninsured.
He states it. It sets the timeline the plan has to beat.
Stated: he works night shifts and sleeps in the morning, and the pantry opens three days a week. The case does not say which hours those are, so the supported finding is a scheduling constraint that has to be verified with the provider, not a proven clash.

The family lost their rental and is in a temporary arrangement.
Stated: he lost his rental, is staying in a relative's garage apartment, and called about shelter paperwork. Whether that paperwork has a deadline is not stated in the case, which is why it is asked rather than recorded.

He lives with diabetes and uses insulin, so ongoing care matters here.
Stated in the case. It affects how quickly a heat illness can become serious and what follow-up he needs.
He states the preference himself. A language-concordant Spanish-speaking professional or a qualified interpreter can both meet it; preferring Spanish is not a deficit, and the point is to confirm suitable support rather than assume which form it takes.

A clinician-recommended follow-up was missed because the trip could not be made.
She states the clinic asked her to return about three months ago and she could not travel. Whether immunizations are due is unknown until the records are reviewed — eight months since a check-up is not on its own evidence of being overdue.

The rescue inhaler is nearly empty and the refill lapsed during the move, and she asks for a qualified Spanish interpreter whenever clinical instructions are explained.
Night symptoms, more frequent inhaler use, and two emergency visits in a month.
She asks for one specifically for medical conversations rather than relying on family. Asthma instructions are detailed, and precision matters more than convenience.

He states that he has had no contact with VA mental health care since the missed appointments and does not know who his provider is now. The missed appointments alone would not establish that; his own statement does.

No prenatal visits at about 32 weeks and no established provider.
Stated directly. Late entry to prenatal care needs prompt clinical contact.
She says she was given Spanish forms she could not read. Treating Spanish as her language is itself a safety failure.

She states she lives with high blood pressure, takes a daily tablet, and has asked whether the heat and the extra water affect it, with no one asked yet. That much is stated. Whether she has a clinician she can reach quickly is not stated, which is why the practiced step is an unanswered medicine question routed to a pharmacist or clinician rather than a care-access finding.
Her age and her high blood pressure are both stated. This is a reason to act today, not evidence that she is unwell.
